A glass of milk can be a childhood favourite, yet for some people it becomes a source of stomach trouble later in life. Bloating, cramps, and diarrhoea can make a familiar food suddenly feel like an unwelcome guest. The surprising truth is that the body has not necessarily “turned against” milk, it may simply be following a normal biological pattern.
Milk contains a natural sugar called lactose. To digest it, our small intestine produces an enzyme called lactase, which breaks lactose into simpler sugars that can be absorbed into the bloodstream. Lactase works like a pair of biological scissors, cutting the large lactose molecule into smaller pieces that the body can handle. Babies produce large amounts of lactase because milk is their primary source of nutrition. For many people, however, lactase production gradually declines after childhood. As enzyme levels fall, lactose is no longer completely digested. Instead, it passes into the large intestine, where gut bacteria ferment it, producing gases and other substances that can lead to bloating, abdominal pain, and diarrhoea. This condition is known as lactose intolerance.
In simple terms, the problem is not necessarily the milk itself, but the body’s reduced ability to process its sugar. When undigested lactose reaches the large intestine, the gut bacteria get to work, almost like tiny chefs preparing a dish that produces rather unpleasant side effects. The result can be gas, discomfort, and an urgent trip to the bathroom. As the saying goes, “what goes in must come out”, but in this case, the journey may be considerably less comfortable.
If this process is so common, why can some adults drink milk without any problems? The answer lies in genetics and human evolution. Around 5,000 to 10,000 years ago, certain human populations began raising cattle and consuming milk regularly.
In these groups, genetic mutations arose that allowed the lactase gene to remain active throughout adulthood, a trait known as lactase persistence. This is a fascinating example of gene, diet, and environment working together over generations.
People who inherited these mutations could continue digesting milk and gained an additional source of nutrition, particularly during times of food scarcity. Milk became more than a drink, it became an extra nutritional resource when other foods were difficult to find. In evolutionary terms, this was like receiving an additional tool in nature’s survival toolbox.
Over generations, this trait became common in many populations of northern Europe and in some pastoral communities in Africa and the Middle East. In these communities, a history of dairy farming helped create conditions in which lactase persistence could provide an advantage. The biology of digestion, therefore, carries an interesting record of human lifestyle and history.
Lactose intolerance can also develop later in life because of illnesses that damage the lining of the small intestine, such as severe intestinal infections, inflammatory bowel disease, or coeliac disease. In these cases, the condition may improve once the underlying problem is treated. Here, the intestine can be thought of as a delicate biological workplace, and when its lining is damaged, its ability to produce lactase may also suffer.
Being lactose intolerant does not necessarily mean giving up dairy completely. Many people can tolerate small amounts of milk, while yoghurt and aged cheeses often contain less lactose. Lactose-free milk and lactase enzyme supplements are also widely available. In other words, it is not always an all-or-nothing affair. For some people, moderation can be the middle path between enjoying dairy and avoiding discomfort.
Different people can also have different levels of tolerance. One person may drink a full glass of milk without trouble, while another may experience symptoms after a much smaller amount. The gut, like people, does not always follow a one-size-fits-all rule.
The next time someone says they have “become” lactose intolerant, remember that their body is not behaving unusually. In fact, from an evolutionary perspective, losing the ability to digest milk after childhood is the norm while drinking milk comfortably throughout adult life is the remarkable exception. The irony is deliciously simple: what many people think of as the “normal” ability to drink milk as an adult is actually the evolutionary exception.
So, if milk no longer agrees with you as it once did, your body may not be breaking the rules. It may simply be following an old biological script written by genetics, evolution, and human history. Sometimes, the stomach has the final word, and it speaks through the language of biology.
